COVID-19 Vaccine Delays Leave Millions of US Children Waiting

Updated COVID-19 vaccines are facing delays for children across the United States as a federal program responsible for providing vaccines to millions of young people has yet to begin distributing the latest doses.

The vaccines were approved for the 2026-27 respiratory virus season in August. However, the Centers for Disease Control and Prevention has not yet opened the ordering process for states through its Vaccines for Children program. The program provides vaccines at no cost to eligible children who are uninsured, underinsured or otherwise unable to afford them.

Federal Vaccine Program Has Not Started Orders

The delay is affecting a significant portion of children in the country. The Vaccines for Children program covers roughly half of US children, making it an important source of access for families who rely on publicly funded immunizations.

State health officials say they have not received the usual instructions needed to begin ordering the updated COVID-19 shots. The situation is particularly notable because commercially insured people can already obtain the new vaccines at some pharmacies.

Pfizer has reportedly started shipping its updated doses, while the federal procurement process for publicly funded programs remains unfinished. The Department of Health and Human Services said officials are still making decisions about vaccine purchases through the Vaccines for Children and Section 317 programs.

Federal officials have also pointed to the unpredictable timing of COVID-19 activity compared with illnesses such as influenza. An HHS spokesperson said the government does not consider an arbitrary September deadline to be a public health requirement for vaccine procurement.

Changing Guidance Adds to the Confusion

The distribution delay comes as federal recommendations for childhood COVID-19 vaccination have changed.

The CDC no longer broadly recommends COVID-19 vaccination for every child. Instead, parents and healthcare providers are expected to discuss the potential benefits and risks when deciding whether a child should receive the vaccine. The change is different from the earlier policy, when vaccination was recommended for everyone aged 6 months and older.

The American Academy of Pediatrics has issued separate recommendations for the 2026-27 season. It recommends vaccination for children between 6 months and 23 months who do not have contraindications. It also recommends vaccination for older children at increased risk of severe COVID-19 and says vaccination should be offered to children ages 2 through 18 when their parents or guardians want them protected.

Families Could Face Unequal Access

Health officials and former CDC leaders have raised concerns that delays in publicly funded vaccine access could create differences between families who can obtain vaccines commercially and those who depend on federal programs.

COVID-19 vaccination rates among children have already fallen considerably. CDC data cited in recent reporting show that fewer than 10% of children between 6 months and 17 years received the previous season’s COVID-19 vaccine.

For families waiting for publicly funded doses, the timing of the rollout remains uncertain. The CDC says it is finalizing procurement decisions and plans to provide additional information.

The delay highlights the challenges surrounding this year’s COVID-19 vaccination campaign, as federal guidance, vaccine availability and recommendations for children continue to change.
